QVI Employee Benefit Highlights

Quality Vision International Inc. (QVI) is committed to providing fair and competitive benefits for all full-time employees. QVI’s Employee Benefits program gives all full-time employees and their families a number of options to choose from selecting benefits that best fit their needs. Although the cost of these benefits is determined by the type of plan(s) chosen, QVI has been able to maintain very affordable rates for employees. 

Excellus Health Insurance: Three Excellus Blue Cross Medical Plans; one co-pay hybrid and two High Deductible Health Plans (HDHP)

  • Three different plans, continuing the good, better and best offering in terms of coverages and costs
  • Coverages are the same, except for the addition of pediatric hearing aids and a $60 eyewear reimbursement to the HDHP
  • HDHP Plans virtually free for single party selections, very low costs to employees on other tiers
  • Coverage options available for significant others/domestic partners
  • Enrollment will become effective on the first of the month after date of hire

Prescription Coverage:

  • $0 cost for Child Generic Brands/under 19 yrs. old
  • 3 tier drug co-pay ($5/$45/$90)
  • RX delivery program (Wegmans or Express Scripts) Pay for 2 months/3rd month is free

KeyBank Health Savings Account (HSA)

  • All HDHP holders are eligible for the KeyBank HSA, Maximum Single $3,400-Maximum Family $6,850 - Over 55 Catch up $1,000
  • No cost to employee for HSA Account as long as online statements are selected, free checking and online bill pay offered
  • Employee can contribute on a pre-tax basis, funds available in bank account on pay day
  • Two investment offerings, thru KeyBank and Health Savings Administrators for excess HSA funds > $3,500 balance
  • Spouse HSA account is free, potential catch-up contribution of $1,000 contribution allowed
  • All contributions exempt from Federal and State income taxes and payroll taxes, very tax efficient

Flexible Spending Account (FSA):

  • QVI offers Medical FSA ($2,600 max)/Dependent-care FSA ($5,000 max)
  • Employee can contribute on a pre-tax basis

Excellus Dental:

  • Pays a portion of dental expenses after deductible is met. Covers preventative, basic and major services.
  • Enrollment will become effective on the 1st of the month after date of hire.
  • Dependents covered to age 26, like Health Insurance
  • QVI covers 2/3’s of cost, annual max benefit up to $1,500 per year, strong Excellus Dental Network

Bonus/Wage Dividend Program:

  • QVI strongly believes in sharing a portion of profits with employees who generate those profits
  • Program looks at the last three years of gross wages, multiplied by a company established factor, profit based
  • Program can vary year to year, with features like vacation time out in lieu of $$, or company contributions to 401-K plan

QVI Profit Sharing Retirement Plan:

  • New employees automatically enrolled/Annual contributions earned with 1,000 hours or more of service and working for QVI on last day of the calendar year
  • Annual Employer Contributions, subject to QVI Board of Directors approval, presently at the 5% of gross wages
  • Employer Contribution Monies invested by a QVI Steering Committee
  • Option to transfer QVI Employer Contribution monies to the QVI 401-K Plan, if employee contributes 5% or more to the 401-K Plan
  • QVI Employer Contributions gradually vest over a 6 year period

QVI 401-K Plan (Fidelity):

  • New employees automatically enrolled @ 3% default contribution rate after 30 days of employment, to the first of the next month
  • Pre-Tax contributions up to 60% of eligible pay, available in 401-K account on pay day, mutual funds already purchased
  • Max contribution of $18,500 (Catch up over age 50 -  additional $6,000)
  • 401-K is a pre-tax savings, not subject to Federal and State Income Taxes
  • You are always 100% vested in your own contributions to the plan, QVI Employer funds gradually vest over a 6 year period        
  • Investment offerings are “all” lower cost institutional share class
  • Employees decide investment decisions and asset allocations

Paid Time Off:

  • 10 paid holidays (6 regular and 4 floaters) - See Calendar for designated days
  • Salaried and hourly employees accrue 2 weeks’ vacation/ end of first year; accrue 3 weeks @ end of 5th year; 4 weeks @ end of 10th year.  Vacation is awarded based on service anniversary date (hire date), several vacation options on use, carryover or payout, decided by the employee.
  • Personal sick time (PST) is allotted at 10 hours per quarter for hourly employees
  • Bereavement Policy per handbook, up to 2-3 days off with pay to ease the pain of losing a loved one

Disability (DBL): QVI offers Short-Term Disability and Long Term Disability thru Mutual of Omaha and New York State (NYS) Disability

  • Employee cost of the NYS Short Term Disability $.60 week- Benefits are 50% of gross wages up to $170 per week
  • Employee cost Enriched Short Term Disability $3.00 per week- Benefits are 70% of gross wages up to $500 per week (includes NYS DBL at $170 per week)
  • Long-term Disability (LTD) is also offered at no cost to the employee- Benefits are a Maximum of $5,000 per month
  • New York State Paid Family Leave (PFL) is also offered to all New York State employees
  • Salaried employees eligible for 12 weeks salary continuation/pay

Employee Assistance Program (EAP):

  • Mutual of Omaha EAP is a confidential, third party administered free program for employees and their dependents should they need assistance with managing the daily stresses of work, home and family.  No cost to employee. 

Tuition Reimbursement/Scholarships:

  • QVI recognizes the importance of supporting the continued knowledge and skill growth of employees thru education/tuition assistance
  • Tuition reimbursement is determined by the employee’s academic performance, up to $5,000 per year limit
  • Dependent Scholarships are available to all tenured employees, thru the Edward C. Polidor Scholarship Program and the John/Joyce Heslink Academic Excellence Program.  See QVI HR for details.

AFLAC Insurance:

  • Employee paid coverages for cancer, accident and hospitalization protection, very affordable rates


  • Additional vision program to augment medical plans, most often used with HDHP
  • Covers exams, frames, contacts and lenses
  • Unmarried dependents are now covered to age 26, like Health Insurance and Dental

Employee Hiring/Referral Bonus:

  • Recommend a friend or family member to join QVI, if hired, referral is worth $1,000, payable in two installments, depending on time served by newly hired employee.  Recommendation must be made at time of application and included in application materials.

Other Benefits:

  • Modern Payroll and Technology platform with Paylocity to access payroll and benefits information as needed
  • Coffee Club $1.25 per week, as much coffee as you would like to drink
  • Gym membership reimbursement, every 6 months, QVI pays $50 to help offset costs
  • Two New York State Notary Publics in QVI Accounting Department
  • QVI Company Store, options to buy QVI logo items at a very competitive cost
  • Rochester Museum and Science Center free admission passes
  • My Better Benefits Program Discounts for movie tickets, car washes, oil changes, other benefits
  • Free Rochester Red Wings box tickets are available on a first come first serve basis, four box seats for every home game
  • Free JPMorgan MasterCard Program to assist employees who need to travel in the conduct of QVI business
  • Shipping of personal items, utilizing QVI negotiated costs/rates with available suppliers like UPS.  Employee expected to reimburse QVI at costs incurred by the company.
  • Pay Advance for vacation/emergencies, one week of pay, available with 4-5 business days prior notice to QVI HR
  • Electric Vehicle (EV) Charging Station Available, free for employees to use/charge their vehicle

QVI Paid Social Events

  • Annual company picnic typically held in September, free food/drink/entertainment for employees and family
  • JPMorgan Chase Challenge Charity event held in Rochester, New York during May/June, includes fees and food/drink

Life and Accident (AD&D) Insurance Plans-Mutual of Omaha/Chubb

  • Basic Life Insurance-Company paid-$35,000 for all employees
  • Optional Term Life Insurance for employees, spouse and children
  • Voluntary Accidental Death & Dismemberment (AD&D)
  • Business Travel/Accident- company provided Life Insurance ($250,000) and Medical Insurance while traveling on QVI business
